Operationalize the holographic memory governance audit caveats found during weekly memory review.

Scope

• Add regression coverage for holographic memory retrieval and governance behavior.
• Ensure fact_store(search) increments retrieval telemetry without degrading hit@1.
• Keep auto_extract fail-closed unless explicitly set to boolean true or string true.
• Add governance data-quality support for stale fact detection and query-log sampling.

Acceptance criteria

• Full memory plugin tests pass.
• Live holographic data-quality check reports ok=true.
• Live holographic audit reports hit@1=1.0 for the curated benchmark.
• Opus 4.8 final review approves the diff.
